Supplier RFP question template for clinics

A clinic's RFP to a compounding pharmacy or sourcing platform should ask, in writing and with documents, who fills the order and how the supplier handles 7 areas: ordering, pricing, accountability, state licensing, turnaround, security and onboarding. This template, dated 26 September 2026, gives 28 questions in 8 groups, a cover note to paste into an email, and a line for each answer.

What to send, and why in writing

A clinic's RFP to a compounding pharmacy or sourcing platform should ask, in writing and with documents, who fills the order and how the supplier handles 7 areas: ordering, pricing, accountability, state licensing, turnaround, security and onboarding. The template below, dated 26 September 2026, gives 28 questions in 8 groups, a cover note to paste into an email and a line for each answer.

The groups follow the 7 criteria of Clinic Partner Guide'sCompounded Sourcing Platform IndexSource 1, which scores 12 platforms from their own public pages.Source 2Public pages leave gaps. Of the 84 criterion cells in the index (12 platforms, 7 criteria), 12 are not stated: 6 in turnaround, 5 in pricing transparency and 1 in security posture (scored 31 August 2026).Source 2Those gaps are where a written question does the most work.

Written answers can be compared line by line, filed and checked later. A sales call cannot.

How to send it

  1. Send the same questions to every supplier.

    Identical questions make the answers comparable. Delete a group only if it does not apply to any supplier on your list.

  2. Fill in the brackets.

    Replace every [bracketed] field with your clinic's details before sending: your volume, your states and the products you are pricing.

  3. Give a reply date and ask for documents.

    Ask for answers in writing, with the documents listed at the end attached.

  4. Record who answered, and when.

    Note the name and role of the person answering and the date of each reply.

  5. Score the replies.

    Use ourvetting scorecard, which applies the same 7 criteria and weights.

A. Your business and who fills the order

Documents to ask for

  • Your current terms, with the version or last-updated date.
  • Your price list, or a written quote at our volume with every fee.
  • The license for each of our states, for each filling pharmacy.
  • The clause that sets out responsibility for late, damaged or wrong orders.
  • A sample order confirmation and a sample invoice.
  • The name and role of the person answering, and the date.

Reading the answers

A blank is information. Mark it unknown and ask once more, in writing. An answer given on a call counts once you write down who gave it and when. The index applies the same rule to a statement a platform confirms directly: it is "recorded with the date and the person who confirmed it".Source 3

Questions clinics ask

What questions should a clinic send a compounding pharmacy or sourcing platform before choosing it?

Written questions on who fills the order and on 7 areas: ordering, pricing, accountability, state licensing, turnaround, security and onboarding. This template gives 28, grouped by the criteria Clinic Partner Guide uses to score 12 sourcing platforms, with a line for each answer and a list of documents to request.

Do I need to send all 28?

Send groups A to E at least: who fills the order, how you order, what you pay, who is responsible and where each filling pharmacy is licensed. Add F to H before you sign.

Should I send these questions to a directory?

A directory does not take orders: all 4 directories in the index score 1 out of 5 on ordering workflow (scored 31 August 2026).Source 2Use a directory to find pharmacies, then send the questions to the pharmacy or platform that would fill your orders.

What if a supplier will only answer on a call?

Write down each answer, who gave it and the date. That is enough to score it, the same rule the index applies to a statement a platform confirms directly.Source 3Then send your notes back and ask the supplier to confirm them in writing.

What each 503A pharmacy partner publishes about pricing

There is no standard 503A compounding pharmacy price for clinics, and the reason is disclosure rather than variation alone: of the 12 sourcing platforms in this index, one publishes actual clinic rates. The rest either describe how pricing works without saying what it costs, or publish nothing about price and quote only after a call. Read as at 2026-08-31.
