How the scorecard works

Score each compounding pharmacy or sourcing platform 0 to 5 on 7 criteria: ordering workflow, pricing transparency and direct accountability at 20% each, and multi-state coverage, turnaround, security posture and onboarding speed at 10% each. Leave a criterion unscored when the supplier gives you no answer, then divide the points by the weight you did score. Before you score

  1. Collect written answers first.

    Send every supplier on your shortlist the same questions, in writing. OurRFP question templategroups them by these 7 criteria. Score from the answers and documents you receive, not from a sales call you did not write down.

  2. Run three pass or fail gates.

    Before scoring, confirm: the business can take and fill your order (or you are using it only to find pharmacies); every filling pharmacy holds a license for your state, which you checked yourself with the issuing state board; and you hold a written price at your monthly volume. A supplier that fails a gate is not scored. It is not ready to compare.

  3. Score each criterion 0 to 5, or mark it U.

    Use the anchors below so two people on your team score the same answer the same way. U means unknown: no answer.

  4. Total it and record the evidence share.

    The steps are under "How to total a supplier". Write the evidence share beside every total.

  • Our methodology publishes each criterion's definition and weight on themethodology pageSource 3; it does not publish a description of each score level. The "Ask for" lines and the anchors below are our suggestion for consistent scoring inside your clinic, not the index's rubric.
  • 5: the written answer covers every "Ask for" item for the criterion, and a document backs it.
  • 4: covers every item, with one gap or with the document still to come.
  • 3: covers about half of the items.
  • 2: covers a small part of them.
  • 1: the supplier answers, but the answer covers none of the items.
  • 0: the supplier states it does not offer this.
  • U (unknown): no answer. A verbal answer counts once you record who gave it and when, the rule the index applies to a statement a platform confirms directly: "recorded with the date and the person who confirmed it".Source 3

How to total a supplier

  1. Multiply each score by its weight.

    20 for ordering workflow, pricing transparency and direct accountability; 10 for the other four. A criterion marked U gets no points and no weight.

A worked example you can check against the index

Points: 5 × 20 + 4 × 20 + 2 × 20 + 5 × 10 + 4 × 10 + 5 × 10 = 360. Weight scored: 90. 360 ÷ 90 = 4.0, which is the overall score the index shows, with 90% of criteria weight evidenced.Source 2

Why the evidence share goes beside every total

Two totals can rest on different amounts of evidence. Across the 12 platforms in the index, the share of criteria weight evidenced runs from 60% to 100% (scored 31 August 2026).Source 2The methodology says so directly: "That does mean two overall scores can rest on different numbers of criteria."Source 3

On your sheet, a supplier that answered 4 of 7 criteria well can outscore one that answered all 7. Read the two numbers together. A low evidence share is a reason to ask again, not a verdict.

What a score does not say

A score on this sheet describes what a supplier told you and could document. It does not assess clinical quality, formulations or outcomes, and it is not legal advice. The methodology puts the same limit on the index: "A platform that discloses little scores lower on transparency, which is a statement about its disclosure and not about its service."Source 3

What criteria should a clinic use to compare compounding pharmacies and sourcing platforms?

Clinic Partner Guide's methodology uses 7: ordering workflow, pricing transparency and direct accountability at 20% each, and multi-state coverage, turnaround, security posture and onboarding speed at 10% each.Source 2This scorecard applies the same criteria and weights to the written answers your clinic collects.

How is the weighted score calculated?

Multiply each 0 to 5 score by its weight, add the points, and divide by the total weight of the criteria you scored. A criterion with no answer is left out rather than scored 0, as in the index's methodology.Source 3Record the weight you scored as the evidence share.

What if a supplier will not answer a question?

Mark that criterion U (unknown), leave it out of the total and note it on the summary sheet. The evidence share will show the gap. Ask again in writing before you decide.

Can I change the weights?

Yes, for your own decision. A multi-state group may weight coverage higher than a single-site med spa would. Write the weights you used on the summary sheet, and do not compare a reweighted total with the index's scores.

What each 503A pharmacy partner publishes about pricing

There is no standard 503A compounding pharmacy price for clinics, and the reason is disclosure rather than variation alone: of the 12 sourcing platforms in this index, one publishes actual clinic rates. The rest either describe how pricing works without saying what it costs, or publish nothing about price and quote only after a call. Read as at 2026-08-31.
